Covid-19 Safety Plan (CSP)

St. John Catholic School, in collaboration with the San Francisco Department of Public Health and the Archdiocesan Department of Catholic Schools, has created the following plan for the reopening of school. All contents are subject to change in deference to local, state, and federal mandates.

To support continued healthy education and the well-being of our families, staff and greater San Francisco community,  St. John has made great efforts to adapt safe and protective procedures on campus. For our complete health and safety information, please refer to our Covid-19 Safety Plan (CSP).

Required Student Re-Entry Forms

Before returning to campus, the following forms must be signed and returned to the school. You may return forms by email or drop off at the school office prior to bringing your student. Thank you for helping St. John educate students safely!

Required Daily Health Check

Parents must conduct a health screening of their child before 7:30 AM each school day, including an at-home temperature check. Parents must then fill out an online form each day for each of their St. John students.

The online health check form is available here. Those who would prefer a physical form can find the paper form here. Temperature checks and review of the forms will happen in the morning walk-up/car line. Temperature checks will be conducted before students exit the care of a family member and at random throughout the day.

Families who have not completed the form online prior to entering school grounds will be provided a paper form. Families will be asked to exit the car line and park in the basketball court area in order to complete the paper form before their child exits their vehicle. Any child experiencing any symptoms or who answers YES to any of the health check questions must remain home and may join their class via the Distance Learning platform.
